Community Health Fund Application

 The NVRH Community Health Fund insures consistent and continuous funding for community health programs and services and confirms the hospital's ongoing committment to community health improvement.

The NVRH Community Health Fund supports a community building approach that goes beyond the delivery of medical care, to improve the health and quality of life for people in the communities we serve.  Community buidling addresses the root cause of health problems such as poverty related issues, as well as identifying and providing services and programs that directly influence health and quality of life.

The application period for 2018/2019 is now open.  Application documents can be downloaded from this page.   

For 2017-2018 NVRH funded:

 Program: EPIC Music Program

Organization: Catamount Arts

Amount: $3000.00

Description: EPIC Music, inspired by Venezuela's youth development and social change program El Sistema, builds student aspirations and fosters resiliency through free music instruction to students in grades 1-5 in St.Johnsbury.

Program:  Kinship Program

Organization: NEK Foster and Adoptive Community

Amount: $3500.00

Description: Northeast Kingdom foster and Adoptive Community has a vision that everyone touched by foster and adoptive care has access to the resources and support necessary to successfully navigate their journey. There is an increased number of Kinship Providers (grandparents, aunts, uncles, etc).  The demands on foster and adoptive families are tremendous and ever increasing. The State Northeast foster and Adoptive Community provides a regular time of support and education to foster and adoptive parents.

Program:  Communities of Color

Organization: St. Johnsbury School

Amount: $3000.00

Description: Families of color are a minority in the Northeast Kingdom of Vermont. They often do not have the opportunity to interact with each other. Communities of Color creates a facilitated space for families to bond and form positive connections with each other as well as our community school. The St. Johnsbury School is collaborating with Sha'an Mouliert to start a Community of Color support and advocacy group. The target population for this project are families of color that attend the St. Johnsbury School.

Program:  The Family Place

Organization: St. Johnsbury School

Amount: $1000.00

Description: The Family Place is a Promise Community program striving to make sure children are ready for school, live in safe and stable homes, have access to high quality early learning experiences, and families have an increase in coherent, effective, collaborative support. The grant will allow the Family Place to purchase incentives and host community meals for Family Place programs.

Program:  Informational Kiosks

Organization: Lyndon Bike and Ped Advisory Committee

Amount: $4500.00

Description: The project is to build two trailhead kiosks to promote walking ahd biking iri Lyndonville. The kiosks will raise awareness of existing trails and better connect Lyndon institute to the downtown and promote walking and biking in our community.
